چکیده
A 28-year-old vegetarian woman with pernicious anemia developed progressive leg spasticity and psychosis leading to food paranoia severe malnutrition. She presented encephalopathy, anemia, hypoalbuminemia, deficiency of zinc vitamins B1, B6, B12. MRI revealed a lesion the splenium corpus callosum (figure 1) spinal cord changes 2). This case shows combination findings associated vitamin deficiencies: (B complex, Marchiafava-Bignami disease)1 (B12, subacute combined degeneration).2 Primary demyelinating, autoimmune, metabolic disorders (Cu, E) were ruled out given clinical context workup. After treatment, resolved; weakness improved.

A 60-year-old man presented with progressive weakness for 2 years that began in the right arm and subsequently spread to all limbs. On examination, he had both upper and lower motor neuron signs including spasticity, hyperreflexia, and fasciculations in addition to asymmetric weakness. The sensory examination was normal. He was diagnosed with amyotrophic lateral sclerosis (ALS). MRI showed bila...

Brijesh P. Mehta, MD Robert H. Shmerling, MD A 57-year-old man with cerebrotendinous xanthomatosis (CTX) was admitted to the hospital after a fall. He had been diagnosed with CTX in his 30s and had had numerous complex-partial seizures, which occur in 50% of adult patients.1 Seizures were characterized by staring spells and speech deficits, occasionally generalizing to tonic-clonic leg movement...

CSF opening pressure was 30 cm of H2O w...

A 65-year-old woman presented with recurrent episodic vertigo lasting hours, accompanied only by headache. While golfing, she experienced transiently blurred vision. She denied arm claudication. Blood pressure was 160/78 in the right arm and 140/65 in the left arm. The left radial pulse was diminished. Cranial nerves, strength, sensation, and coordination were normal. Maneuvers to provoke sympt...
متن کاملMultiple endobronchial mass lesions due to sarcoidosis.
Sarcoidosis is a systemic granulomatous disease that primarily affects the lung and lymphatic systems of the body. The lungs are affected in over 90% of sarcoid patients. Although paranchymal lung disease is more common, the airways may also be involved. Bronchial mucosa is often affected in sarcoidosis, but endobronchial mass lesions are very rare. We present a case with multiple endobronchial...
